An appropriate assessment helps us understand psychological, behavioural, medical and social needs, along with the person’s strengths and goals.
A plan is developed from the assessment and professional recommendations. We discuss the options, expected steps and the person’s role in decisions.
The person takes part in suitable psychological, behavioural, medical or rehabilitation interventions according to the care plan.
Progress is reviewed at appropriate intervals. If needs change, the plan can be adjusted in discussion with the person and relevant care professionals.
When a programme is ending, discharge planning can support the transition back into daily routines, relationships and community life.
Follow-up, relapse-prevention planning and continued support may be arranged according to the individual’s needs and care plan.
Our commitment
Admission to a programme follows appropriate assessment, clinical suitability, informed consent and applicable professional and legal requirements. We take time to explain the process and discuss concerns with the person and, where appropriate, their family.
